The ESI file uses UDINT “format”. This means that I cannot send negative values. Would it be possible to get it in REAL?

No, you have to typecast your data accordingly to USDINT. Then and the other side you have to typecast this data, i.e. to REAL.

Would it be possible to get an ESI file with 256 bytes output / input for GW EtherCAT?

Yes, this file provides 256 bytes for output and input
